Comparison Between Ejection Fraction and Left Ventricular Volume According to Perfused Myocardial Images Synchronized to ECG (Gated Spect) and Cineventriculography

Electrocardiography gated myocardial perfusion single photon emission computed tomography (GSPECT) with99mTcMIBI or201T1offers the potential to simultaneously assess IV function and myocardial perfusion with just one acquisition procedure. The purpose of this study was to compare results of GSPECT with those of angiographic ventriculography in the assessment of left ventricular ejection fraction (LVEF), left ventricular end diastolic volume (LVEDV)and left ventricular end systolic volume (LVESV). Fifty-one patients (35men, mean age65 years)underwent GSPECT and angiographic ventriculography within 30 days. GSPECT resulted in a lower mean LVEF (%) (-2.67 ±6.2, p < 0.05) and higher LVEDV (23.7 ± 36ml, p<0.05) and LVESV (15.5 ± 25 ml, p < 0.05) compared with angiographic ventriculography. The correlation between the two methods was good for LVEF (r= 0.89), LVEDV (r = 0.74) and LVES (r = 0.85). There was also significant concordance between both techniques in the estimation of LVEF in patients with EF greater than 50% or smaller than this percentage (kappa = 0.70) and an acceptable concordance to estimate LVEDV (kappa 0.44) and LVESV (kappa 0.56).
Conclusions
GSPECT is an accurate method for assessment of left ventricular function. The correlation with left ventricular angiography is good. GSPECT tends toa slight underestimation of LVEF and over estimation LVEDV and LVESV compared with angiographic ventriculography.
