A 70 years-old man, with history of hypertension, had a stroke five days after surgical remove of colon adenocarcinoma. Transthoracic echocardiography showed no abnormalities, later the transesophageal detected a large and mobile thrombus attached to the wall of an atheromatous ascending aorta (debris). The patient was started to conventional anti-coagulant treatment and eight month later a repeated transesophageal echocardiogram verified the lysis of the thrombus without embolic events. This report emphasize the transesophageal echocardiography role in detecting aortic source of embolism, the efficiency of anticoagulant treatment anda possibility of paraneoplasic syndrome.
